Uninstall OutlookReminder Custom from from Microsoft Outlook

I’ve currently had to uninstall the program but the tab “reminder call” between the scheduling and appointment tab that the program inserted remains. I would like to eliminate this tab. Please give me instructions to this end. Thanks

Select Tools > Options… > Other > Advanced Options… > Custom Forms…
then delete the ReminderCall form.
